Breast MRI vs. mammography
Breast MRI complements conventional breast diagnostics and offers advantages when other procedures reach their limitations. It is primarily used when particularly detailed, high-resolution images of breast tissue are required – for example in women with dense glandular tissue, in cases of unclear findings from ultrasound or mammography, or for monitoring women with a family history of breast cancer.
Unlike mammography, MRI does not use X-rays but instead works with magnetic fields and radio waves. This makes it particularly suitable for younger women or patients for whom radiation exposure should be avoided.

What happens during the examination?
For this specialized examination of your breasts, we use the MAGNETOM® Skyra MRI scanner from Siemens Healthineers, which combines modern high-field technology with a field strength of up to 3 Tesla. This radiological procedure provides particularly high-resolution, high-contrast images of breast tissue, enabling us to detect even the smallest tissue changes.
The examination is performed in several steps:
Preparation
First, we ask you to remove metallic objects such as jewelry and piercings, as these can interfere with the magnetic field of the MRI scanner.
To visualize the breast tissue, we administer a contrast agent through a vein in your arm. This highlights areas with increased blood flow – as may occur in tumors – particularly well. If you have any known allergies or kidney disease, please let us know in advance. We will adapt the examination accordingly.
Examination
During magnetic resonance imaging, you lie on your stomach on a special positioning unit with openings for your breasts so that they can be positioned freely without pressure.
The padded examination table is then slowly moved into the MRI scanner. The actual scan takes approximately 20 to 30 minutes. During this time, detailed cross-sectional images of both breasts are produced in several sequences. It is important that you remain as still as possible to ensure the best possible image quality.
After the examination
After the imaging procedure, you can get dressed immediately and resume your usual activities. The contrast agent is excreted through your kidneys.

What can magnetic resonance imaging detect?
Breast MRI can visualize a wide range of changes in breast tissue with great precision. The examination shows not only the anatomy but also the blood supply to the tissue – an important advantage when distinguishing between benign and malignant changes.
Typical situations in which breast MRI is used include:
Detecting cancer at an early stage is crucial for successful treatment. In women with an increased familial risk – for example due to confirmed genetic changes such as BRCA1 or BRCA2 – breast MRI is often used as part of regular preventive examinations. In these cases, the lifetime risk of developing breast cancer is significantly increased.
High breast density means that the breast contains a large proportion of glandular and connective tissue and less fatty tissue. This is a normal anatomical variation and is not unusual or harmful in itself.
However, dense tissue presents a diagnostic challenge in mammography: both healthy glandular tissue and tumors appear white on the X-ray image. This can make abnormalities more difficult to detect or assess clearly.
MRI offers important advantages in this situation. Contrast-enhanced visualization of blood flow can make even very small tumors visible – often before they can be felt or detected by ultrasound or mammography.
Sometimes an X-ray examination of the breast or an ultrasound does not provide clear results – for example when an abnormality is visible but cannot be clearly classified as benign or malignant. MRI provides particularly detailed images that allow the spatial extent of the tumor to be assessed very accurately. It shows not only the size of the tumor, but also whether it has branched, whether several lesions are present (multifocal or multicentric disease), or whether neighboring tissue is affected. MRI is often superior to mammography and ultrasound, particularly in lobular carcinoma – a specific type of cancer that often spreads inconspicuously through the tissue.
Residual tumor tissue remaining in the breast after previous treatment, such as chemotherapy, can also be visualized particularly reliably with MRI. This allows surgery to be planned precisely while preserving as much healthy tissue as possible.
Overall, preoperative MRI helps avoid over- or undertreatment, allows the surgical strategy to be tailored individually and may support breast-conserving treatment where appropriate.
Regular follow-up after breast cancer treatment is particularly important in order to detect possible recurrences at an early stage. These may occur in the previously affected breast, in the scar area or, following breast-conserving surgery, in adjacent tissue.
MRI plays an important role in follow-up after breast cancer, particularly when other imaging procedures do not provide clear results because of existing scar tissue, implants or dense breast structures.
Examples of situations in which MRI is used during follow-up include:
- unclear palpable findings or symptoms in the surgical area
- suspected recurrence that cannot be clearly assessed with other imaging procedures
- follow-up in patients with a very high risk of recurrence, e.g. due to genetic predisposition
- monitoring of reconstructed breasts or implants where other procedures provide limited information
Thanks to its high level of accuracy, breast MRI helps provide clarity at an early stage and gives both you and your treating physicians additional confidence when planning the next steps.
Breast implants may change or become damaged over time – for example due to material fatigue, external influences or natural aging processes. An implant can leak or become deformed without you noticing immediately.
MRI is currently the most accurate procedure for detecting silent, asymptomatic implant ruptures – in other words, ruptures that do not cause pain or externally visible changes.
Typical reasons for performing an MRI of breast implants include:
- suspected rupture or leakage of an implant
- clarification of symptoms such as feelings of tension, swelling or deformation
- regular monitoring of older implants to detect material changes at an early stage
- assessment of implant position following breast surgery or accidents
Unlike mammography, MRI is particularly gentle for women with implants because the implant is not compressed during the examination. Imaging is performed at very high resolution, enabling us to detect even very small tears or changes. MRI therefore makes an important contribution to the safe long-term monitoring of breast implants.
Breast-conserving treatment or other breast surgery naturally causes changes in the tissue. These include scar formation, tissue hardening and smaller lumps. These changes are generally harmless but can be difficult to distinguish from possible tumor recurrence during follow-up – particularly on mammography or ultrasound.
MRI offers important advantages here. It provides functional imaging in which a contrast agent allows us to visualize the blood supply to a specific area of tissue. Tumor tissue generally shows significantly stronger contrast enhancement than inactive scar tissue, which usually has a poor blood supply and therefore appears differently.
MRI can be useful for differentiation in cases such as:
- unclear tissue changes in the surgical area that cannot be clearly assessed by ultrasound or X-ray
- palpable lumps at the scar where it is unclear whether the finding represents scar tissue or recurrence
- follow-up examinations in patients at high risk of recurrence to detect changes at an early stage
