What is prostate MRI?

This is a specialized form of magnetic resonance imaging used for detailed visualization of the prostate. Magnetic fields and radio waves are used to produce high-resolution cross-sectional images of the prostate tissue – entirely without X-rays.

The so-called multiparametric prostate MRI (mpMRI), which we offer at our private practice and preventive care center in Düsseldorf, is particularly valuable. It combines several imaging sequences. This allows the structure, blood flow and metabolism of the prostate tissue to be visualized precisely. As a result, we can detect and reliably assess even very small changes.

mpMRI at our private radiology practice in Düsseldorf provides a detailed view of the prostate gland and adjacent tissue. This allows us to reliably assess both its external shape and size and finer details within the gland.

These include:

  • the different areas within the prostate (central, peripheral and transition zones)
  • the seminal vesicles and their connecting ducts
  • important neighboring structures such as the urethra, pelvic floor and surrounding fatty tissue
  • regional lymph nodes and blood vessels that are important when assessing possible tumor spread

In addition, mpMRI provides precise imaging in several planes, creating a comprehensive three-dimensional view.

Which changes can be detected with prostate MRI?

MRI of the prostate can reveal differences in tissue structure. This makes it possible to distinguish inflammatory processes as well as benign and malignant changes.

These include:

Benign prostatic hyperplasia (BPH)

Benign enlargement of the prostate is often troublesome for those affected, but is not dangerous. On MRI, the glandular tissue generally appears enlarged with nodular or irregular structures that can be clearly distinguished from malignant changes.

Prostate cysts

These are fluid-filled cavities that are also generally harmless. MRI shows them as smooth, fluid-filled structures that can be clearly distinguished from the surrounding prostate tissue.

Prostatitis

Inflammation of the prostate can cause various symptoms but is usually treatable. During the examination, the tissue often appears less homogeneous and shows poorly defined changes. Increased contrast enhancement due to greater blood flow is also characteristic.

Abscesses

These collections of pus within the prostate tissue need to be taken seriously, but can usually be treated successfully with targeted therapy. On imaging, they appear as fluid-filled cavities with a thick, irregular capsule. Abscesses are clearly separated from the surrounding tissue and show peripheral enhancement after contrast agent administration. This makes them easier to distinguish from other changes.

Prostate cancer

A malignant tumor of the prostate appears as a darker area within the tissue on MRI images. It also shows altered movement of fluid within the cells and increased contrast enhancement in certain regions. The course of the disease depends greatly on how early it is detected and how aggressively the tumor grows.

Infiltration of surrounding structures

If a tumor has already spread beyond the prostate, this can also be clearly identified using mpMRI. Typical signs include involvement of the seminal vesicles, extension toward the urinary bladder or pelvic floor, or growth beyond the natural capsule of the prostate. On imaging, the borders of the prostate capsule may appear poorly defined or interrupted. Tumor extensions into adjacent tissue or corresponding signal changes in neighboring organs may also be visible. Such findings can indicate a more advanced stage of disease and are important when selecting the appropriate treatment.

Enlarged lymph nodes

Enlarged lymph nodes can be another indication that tumor cells have spread beyond the prostate. They may appear significantly enlarged or structurally altered compared with healthy lymph nodes. Such findings are important because they may indicate possible metastasis and can therefore have a major influence on further treatment planning and prognosis.

What happens during the examination?

We use the MAGNETOM® Skyra MRI scanner from Siemens Healthineers. This is one of the most advanced MRI systems and has a particularly high field strength of 3 Tesla. This enables exceptionally clear and detailed visualization of the prostate and its fine structures. The advanced technology makes even very small tissue changes visible that may be difficult to detect with less powerful equipment.

The examination is performed as follows:

Preparation

If necessary, you may be given a mild laxative or enema to minimize interference from the bowel. Depending on the diagnostic question, we may administer an intravenous contrast agent, which can help visualize blood flow within the prostate and make suspicious areas more clearly visible.

Examination

For the scan, you lie comfortably on our MRI examination table. Thanks to the wide opening of the scanner, the examination is also more comfortable for patients with claustrophobia. To minimize movement and ensure optimal visualization of the prostate, we use gentle support around the abdominal and pelvic area.

The examination table is then slowly moved into the MRI scanner. The examination takes approximately 30 to 45 minutes, during which the scanner records several different imaging sequences.

After the examination

Once the examination is complete, you can leave the scanner and get dressed. We carefully evaluate the images.

For screening / early detection of prostate cancer

Multiparametric MRI of the prostate enables particularly precise assessment of prostate tissue – even before symptoms occur. It is used in men with an increased risk or an abnormal PSA level and can help detect suspicious areas at an early stage. Compared with other procedures, the examination provides considerably more detailed information.

To determine whether a biopsy is necessary

If the PSA level is elevated or a physical examination produces an unclear finding, the question often arises whether a tissue sample is necessary. In such cases, multiparametric MRI can help identify suspicious changes or confirm an unremarkable finding. This can help avoid unnecessary biopsies.

Localization of tumors before tissue sampling

If a prostate biopsy is required, multiparametric MRI enables precise localization of suspicious areas within the tissue. Instead of taking multiple samples without precise targeting, the abnormal regions can be identified accurately. This significantly improves the accuracy of the biopsy and increases the likelihood of reliably detecting a tumor at an early stage while reducing the burden of the procedure.

Assessment of tumor spread in known prostate cancer

If prostate cancer has already been diagnosed, multiparametric MRI helps determine the precise extent of the tumor. It shows whether the cancer is confined to the prostate or has already affected adjacent tissue, the seminal vesicles or lymph nodes. This information is crucial when selecting the most appropriate treatment.

Follow-up during Active Surveillance

MRI also plays a central role in patients with low-risk prostate cancer that is initially monitored closely rather than treated immediately. It enables reliable assessment of whether the tumor changes or spreads over time. This can help avoid unnecessary procedures without compromising medical safety. MRI complements regular PSA monitoring and helps determine the appropriate time for treatment if intervention becomes necessary.

Detection of recurrence

Following previous treatment, for example surgery or radiotherapy, prostate MRI can help detect possible recurrence at an early stage. If the PSA level rises again after treatment, magnetic resonance imaging provides precise images that can reveal local recurrence and distinguish it from scar tissue or treatment-related changes. This provides a reliable basis for planning the next steps.
