Memory problems, reduced concentration or changes in thinking can be early signs of dementia. The term does not describe one single disease but covers a variety of causes and patterns of progression, including Alzheimer’s disease and vascular dementia. This makes early and precise assessment of possible changes particularly important. Alongside clinical consultations and cognitive tests, imaging procedures play a decisive role in diagnostics. They can make structural changes in the brain visible and provide indications of the underlying form of dementia. At our radiological prevention center in Düsseldorf, we use high-resolution magnetic resonance imaging for this purpose.
The term “dementia” is an umbrella term for different diseases that all lead to a gradual loss of cognitive abilities.
More than 50 forms of dementia are known. Regardless of the particular form, similar changes often occur: memory declines, thought processes slow down and other cognitive abilities are impaired. Many affected people also experience additional changes in behavior, personality or social interaction.
According to the German Alzheimer Society, around 1.7 million people over the age of 65 in Germany are living with a form of dementia.
These are forms in which dementia itself is the primary disease. The causes lie directly in the brain: neurodegenerative processes lead to the progressive loss of nerve cells. Abnormal protein deposits often play a role, impairing nerve cell function and promoting their degeneration.
The most important forms of primary dementia include:
In this form of dementia, certain proteins, particularly beta-amyloid, accumulate in the brain. These changes usually begin in the hippocampus and spread from there. Typical features include increasing memory problems, impaired cognitive abilities and changes in behavior.
This form develops as a result of circulatory disorders in the brain. Damage to the blood vessels, for example from long-standing high blood pressure, means that certain areas of the brain are no longer supplied adequately. Concentration problems, memory difficulties and impaired thinking are common.
In this form, specific protein structures known as Lewy bodies accumulate in nerve cells. Unlike Alzheimer’s disease, these deposits do not consist of beta-amyloid but primarily of the protein alpha-synuclein.
Lewy body dementia is characterized less by isolated memory problems and more by marked fluctuations in cognitive performance. Affected people may appear clearly oriented on some days and considerably impaired on others. Visual hallucinations and motor symptoms resembling Parkinson’s disease also occur frequently.
In this rarer form, the frontal and temporal lobes are primarily affected. The disease often presents less with memory problems and more with changes in personality, behavior and language.
Secondary dementias do not develop as an independent disease of the brain but as a consequence of other medical causes. The cognitive impairments are therefore a manifestation of an underlying disorder rather than the primary condition. This is also an important point of intervention: if the cause is identified and treated early, symptoms can often be influenced positively or their progression slowed.
Precise diagnostics are central here. Imaging procedures such as MRI can make structural changes in the brain visible and provide clues to possible causes, helping guide further treatment.
Possible causes of secondary dementia include:
In advanced HIV infection, the central nervous system can be damaged. This often leads to declining memory, concentration problems and impairments in language and coordination.
Long-term excessive alcohol consumption can damage the brain. Affected people often show memory problems, impaired thinking, behavioral changes and reduced motor coordination. This is sometimes colloquially referred to as “alcohol-related dementia”.
Cognitive impairment can also occur after a severe head injury, for example following an accident. This may range from concentration difficulties to pronounced changes in memory and personality and depends greatly on the extent of the damage.
Other factors such as infections, hormonal disorders or side effects of certain medications can also lead to secondary dementia. Comprehensive assessment is therefore important in order to identify treatable causes at an early stage.
In clinical practice, the various forms of dementia cannot always be clearly separated. Different disease processes often occur at the same time and influence one another. A particularly common example is the combination of Alzheimer’s disease and vascular dementia, in which typical symptoms of both forms overlap.
Other neurodegenerative diseases may also be involved, such as Parkinson’s disease or Lewy body dementia. This can make the clinical picture more complex and less clear.
Differentiated diagnostics are therefore especially important. Modern imaging procedures such as MRI can help visualize and better classify different changes in the brain. Because mixed forms can vary considerably in severity, every situation requires an individual assessment and a diagnostic approach tailored to the findings.

After the medical consultation, standardized cognitive tests are used to provide an initial assessment of mental performance. If the suspicion becomes stronger, referral to specialized disciplines such as neurology usually follows.
To further confirm the diagnosis and determine the exact form of the disease, imaging is a central component. Neuroradiology in particular can visualize structural changes in the brain and support differentiated interpretation of the findings.
Within imaging diagnostics for dementia, MRI of the head is the state-of-the-art procedure. It provides a particularly detailed view of brain structure and important information about dementia-related changes, often at an early stage.
Typical MRI findings can include a reduction in brain volume, changes in the hippocampus or indications of circulatory disorders and vascular changes. These structural patterns can help differentiate between different forms of dementia.
MRI can also help rule out other causes of cognitive impairment, such as tumors, inflammation or structural abnormalities, and therefore supports a well-founded diagnostic assessment.
Many patients ask whether MRI, for example as part of a comprehensive health check-up, can be useful for the early detection of dementia syndromes.
MRI can contribute to an individual risk assessment. It can reveal structural changes in the brain that may indicate early processes, such as vascular changes or initial signs of neurodegenerative developments.
As part of modern preventive concepts such as whole-body MRI, the brain can also be included in the examination. At the Radiology Düsseldorf prevention center, we offer such examinations to provide a comprehensive picture of your health and identify potential abnormalities at an early stage.
This can be particularly useful when there is an increased personal risk, for example because of a family history. MRI may also be a valuable addition for patients who want to take an active role in preventive healthcare.
However, it is important to keep the results in perspective: MRI cannot reliably predict whether dementia will develop. Instead, it provides additional information about the current condition, which must be assessed together with the medical history, clinical examination and other findings.
